In recent years, significant progress has been made in IVF technology, and the success rate has also greatly improved. For infertile couples and doctors, striving for a higher success rate is natural. However, simply pursuing success rate cannot fully reflect the effectiveness of IVF. In this regard, the oocyte utilization rate, as a new evaluation indicator, provides a more comprehensive perspective, helping to select appropriate ovarian stimulation protocols and improve the overall treatment effectiveness.

 

During the IVF treatment process, each oocyte has less than a 7% chance of eventually becoming a live birth. There are several reasons for the significant waste of oocytes. Firstly, in any IVF retrieval cycle, only a small fraction of oocytes have developmental potential to eventually become live births. Naturally, only one dominant follicle becomes the leading follicle in each menstrual cycle, while the rest of the follicles naturally regress. During ovarian stimulation, these follicles that would otherwise be eliminated are salvaged, but these oocytes may themselves have abnormalities or be in a degenerative stage. Secondly, as women age, the number of oocytes with live birth potential decreases. Increasing age leads to a significant increase in the number of chromosomally abnormal embryos, and the probability of live birth per oocyte significantly decreases. For young women with good ovarian function who need IVF due to tubal problems or male factors, their oocyte quality is usually good, and even two children can be born with 2 to 5 oocytes. However, during IVF stimulation, a large number of oocytes are retrieved, resulting in surplus embryos being cryopreserved in liquid nitrogen tanks for long periods, leading to resource waste.

 

To improve the oocyte utilization rate, the following measures can be taken. Firstly, reduce the number of oocytes obtained by advocating the use of mild stimulation and micro-stimulation protocols. The number of oocytes obtained in a conventional stimulation cycle is usually between 8 and 12, while in a micro-stimulation cycle, it averages between 2 and 3, and in a mild stimulation cycle, it averages between 5 and 8. By obtaining a small number of high-quality oocytes, the ideal implantation rate can still be achieved, effectively reducing oocyte waste. Secondly, increase the proportion of oocyte freezing.
